Intensive Case Management Services (ICMS)
Trauma-informed, wraparound support
First Day's Intensive Case Management Services (ICMS) program provides comprehensive, individualized support to adults and families experiencing homelessness or housing instability. Guided by a Housing First philosophy, the program helps participants obtain and maintain permanent housing while addressing the barriers that can impact long-term stability. Through trauma-informed, person-centered case management, ICMS empowers participants to build independence, improve overall well-being, and create a foundation for lasting success.
Participants are referred through dedicated case managers at Pathway Home, who work collaboratively with each individual to develop personalized goals and service plans. Our ICMS program offers ongoing housing stabilization services, benefits advocacy, connections to physical and behavioral healthcare, employment and educational resources, financial literacy, life skills development, transportation assistance, crisis intervention, and referrals to community-based services. Case managers also partner closely with landlords, property managers, healthcare providers, and other community organizations to promote successful housing placement and long-term retention.

At the heart of the ICMS program is a commitment to building trusting relationships and providing holistic support that addresses the unique needs of each participant. By combining individualized case management with strong community partnerships and ongoing advocacy, First Day helps participants overcome barriers to housing and self-sufficiency. Since launching in April 2025, the program has successfully housed 74 individuals, reflecting its continued commitment to creating lasting housing solutions and empowering participants to thrive within their communities.
